\u2013 Conservative treatment is generally ineffective<\/strong>, which underscores the importance of surgical intervention<\/strong>.
\u2013 Surgical treatment is challenging<\/strong> due to the severity of degenerative-dystrophic changes<\/strong>.
\u2013 There is no universally accepted surgical treatment algorithm<\/strong>.<\/p>\n\n\n\n

Surgical treatment of lumbar scoliosis aims to achieve the following goals:<\/strong><\/p>\n\n\n\n

    \n
  • Relief of clinical and neurological symptoms of the disease<\/li>\n\n\n\n
  • Correction of spinal deformity and restoration of anatomical relationships in the lumbar region<\/li>\n\n\n\n
  • Prevention of further progression of the deformity<\/li>\n<\/ul>\n\n\n\n

    According to these goals, surgical treatment involves:<\/p>\n\n\n\n

      \n
    • Adequate decompression of the spinal canal and nerve structures<\/li>\n\n\n\n
    • Stable fixation and correction of the lumbar spine using modern metal constructs<\/li>\n\n\n\n
    • Creation of posterior and interbody spinal fusion (spondylodesis)<\/li>\n\n\n\n
    • Restoration of the height of the intervertebral space and foraminal openings<\/li>\n<\/ul>\n\n\n\n

      Indications for surgery:<\/strong><\/p>\n\n\n\n

        \n
      • Persistent lumbar pain (lumbalgia), lumbar sciatic pain (lumboischialgia) that does not respond to conservative treatment<\/li>\n\n\n\n
      • Myelogenic intermittent claudication<\/li>\n<\/ul>\n\n\n\n

        Clinical case:<\/strong>
        Patient, 13 years old. Lumbar scoliosis, Grade III. Degenerative-dystrophic changes in the lumbar spine. Myeloischemia, symptoms of intermittent claudication.<\/p>\n\n\n\n

        Before surgery: 35\u00b0<\/strong>
        Combined spinal canal stenosis due to protrusion and thickening of the ligamentum flavum.<\/p>\n\n\n\n

        After surgery:<\/strong>
        Operation included:<\/p>\n\n\n\n

          \n
        • Mobilization of the deformity using the Shulutko method<\/li>\n\n\n\n
        • Partial facetectomy<\/li>\n\n\n\n
        • Deformity correction and fixation with a transpedicular frame system<\/li>\n\n\n\n
        • Autospinal fusion (autospodylodesis)<\/li>\n<\/ul>\n\n\n\n

          After surgery:<\/strong>
          100% correction of the deformity.
