In the case of obvious or reasonably expected progression of deformity, the use of specialized orthopedic corsets designed to correct pathology and prevent further development of the disease is the cornerstone of conservative therapy. Obvious progression refers to the development of scoliosis greater than 50° within six months. The indication for corset therapy for dominant thoracic curvature lies within the range of 10°-40°, and for dominant lumbar scoliosis, it is between 10°-30° according to the Cobb angle.

In exceptional cases, for younger children with sufficient flexibility and deformities greater than 40°, corset treatment can also be considered. In cases of clear or reasonably expected deformity progression, it is advisable to begin corset therapy even for curvatures less than 10°. The less severe and more mobile the initial curvature, the greater the correction that can be achieved. Therefore, timely prescribing of a corset not only helps to stop the progression of the curvature but also ensures significant three-dimensional correction of the scoliotic deformity of the spine.

The Sheno Corset: A High-Tech Solution

One of the most well-known and advanced corsets in this field is the dynamic Sheno corset, developed over 40 years ago by French military physician Jacques Cheneau. The corset is a rigid structure made from specialized thermoplastic materials. Inside this structure are precisely calculated protrusions (pelots), designed using a computer program. These protrusions apply the necessary mechanical pressure to the deformed parts of the bone structure, thereby correcting the spine. On the opposite side of each protrusion are expansion cavities for the displaceable tissues.

The Sheno corset cannot be purchased ready-made; it is custom-designed and made to order for each individual patient. The length of time the corset should be worn is determined by the doctor, taking into account the patient’s age, severity of the deformity, and whether the condition is progressing or stabilizing.

Contraindications for Wearing the Sheno Corset:

The use of the Sheno corset is not recommended under the following circumstances:

Organic diseases of the central nervous system (CNS) and psychological or emotional intolerance to the method of treatment.

Scoliosis of less than 20° in patients at the stage of completion of bone growth (Risser stage IV or higher on radiographic tests).

Severe chronic functional disorders in the respiratory and cardiovascular systems.

Degenerative changes in the musculoskeletal system that prevent mechanical influence.

Muscle atrophy, where mechanical pressure is contraindicated.

Skin diseases on the trunk or pelvis with inflammatory foci (such as eczema, dermatitis, or purulent infections) at the sites where the corset would contact the skin.
